Do not enter / provide personal information
The Web site provides information on obtaining reimbursement for certain prescription medications through the various drug programs and drug plans available to Canadians. In some instances the forms required to obtain such reimbursement are made available on the Web site. You may download these forms for use in your healthcare provider practice in accordance with our Terms of Use. These forms should only be populated with patient and prescriber information after the form has been downloaded and the computer is disconnected from the Web site and from the Internet. Except as set out below, Plasmid does not collect and will not accept any communication that includes personal information, including personal health information.
Collection of User Information
We may automatically collect general anonymous information related to your use of the Web site, such as the date and time a user visits the Web site and the pages visited on the Web site. Our servers may automatically collect information about the type of browser you use and the Internet protocol (IP) address.
